Permission Slips

• District Consent/Release - (English/Spanish) This form covers all types of media and only needs to be signed by the parent once. Once it is signed, sending home an informational letter regarding the videoconference should suffice.

• Student Permission Form - Use this form if the District Consent/Release form has not been signed. Once per year is sufficient. After the initial permission slip is signed, please send home an informational letter regarding any additional videoconferences.
