Immunization Requirements
Students enrolling in the Dallas Independent School District for the first time (PK, KN, students from another state or country) must provide evidence of current Texas required immunization compliance by submitting a record from the physician or health clinic.
Exemptions:
Exemption from immunization requirements may be granted for medical or religious reasons. Parents may request in
writing from the Texas Department of State Health Services a waiver and provide the original notarized document to the
campus administrator before the child can be enrolled in school.

A 30-day provisional enrollment is available to students
- who transfer from one Texas school to another Texas school, and
- who are defined as homeless according to the federal McKinney-Ventro Act 42 U.S.

Required Immunizations:
- DIPHTHERIA TETANUS PERTUSSIS – Students under age 7, four doses of the vaccine with one after the fourth birthday. For students age 7 and older: three doses of vaccine with one dose after the fourth birthday and every 10 years thereafter.
- POLIOMYELITIS – Three doses of the vaccine with one dose after the fourth birthday.
- RUBEOLA (Measles) – Two doses of the vaccine, with one after the first birthday or serologic confirmation of immunity.
- RUBELLA (German Measles) – One dose after the first birthday or serologic confirmation of immunity.
- MUMPS – One dose since the first birthday or serologic confirmation of immunity.
- HAEMOPHILUS INFLUENZA TYPE B –For students through 4 years of age, one dose of vaccine after 15 months of age, or a series of three doses with one dose received after 12 months of age and 8 weeks between the last 2 doses. Serologic confirmation of Hib immunity is acceptable in lieu of immunization dates.
- HEPATITIS B – Three doses.
- VARICELLA – One dose received on or after the first birthday. For students who started varicella immunization after age 13, two doses are required. Parents or physician-validated history of varicella disease (chicken pox) is acceptable in lieu of an immunization dates.
- PNEUMOCOCCAL – (age 24 months to 59 months) One dose after age 24 months or a minimum of three doses with one after 12 months of age.
- HEPATITIS A – Two doses are required for students age 24 months through prekindergarten. For students in kindergarten through 12th grade, two doses are recommended but not required.
Recommended vaccines (consult your physician):
- Meningococcal Vaccine – One dose recommended to student’s ages 11-12 years and to previously unvaccinated adolescents at 15 years. Other high-risk groups and younger children may be eligible for the vaccine.
- Human Papillomavirus vaccine – Two doses series with the first dose to females at 11-12 years and the second dose two months after the first dose. Adolescent females ages 13-18 years may receive the series if previously not vaccinated.
- Influenza vaccine – Two doses for children ages 5-9 years receiving the vaccine for the first time. One dose for children ages 5-21 receiving the vaccine after previous year(s) of vaccinations.
