The Choice school application for the 2019-2020 school year is closed.

Important Notes:

• Prior to submitting your responses, please verify you’ve signed up for the correct transformation school, along with the student’s grade level in the 2018-2019 school year.
• Once you submit your student’s information, you will receive a copy of your responses via email. Please retain a copy of your responses for you records.
• Due to the high volume of responses, we are unable to provide you with your waitlist number.
• If a seat becomes available for your student, you will receive a call directly from our office. At the end of the school year, your student will automatically roll over to the next grade level's waitlist.

• Si un asiento está disponible para su estudiante, recibirá una llamada directamente de nuestra oficina. Al final del año escolar, su estudiante pasará automáticamente a la lista de espera del siguiente nivel de grado.
