How to Report an Incident involving Students, Parents or Visitors (No Employee's)

    An “Incident” is an injury to students, parents, or visitors, as well as any physical loss involving Dallas ISD schools, facilities, and equipment.  An incident is NOT an employee injury.

    How to report an incident:

    one Go to Oracle
    two Enter your user name and password
    three Click on “DISD Risk Management Portal”
    four Click on “Create Incident”
    five Complete form by filling in all required fields
    six If the incident includes property/content loss, complete “Property Loss Content Information”
    seven Once all applicable fields are completed, click on “Submit E-Mail”
    eight A copy will immediately be sent to RiskManagement@dallasisd.org
