How to Report an Incident involving Students, Parents or Visitors (No Employee's)

An “Incident” is an injury to students, parents, or visitors, as well as any physical loss involving Dallas ISD schools, facilities, and equipment. An incident is NOT an employee injury.
How to report an incident:
Go to Oracle Enter your user name and password Click on “DISD Risk Management Portal” Click on “Create Incident” Complete form by filling in all required fields If the incident includes property/content loss, complete “Property Loss Content Information” Once all applicable fields are completed, click on “Submit E-Mail” A copy will immediately be sent to RiskManagement@dallasisd.org
