• Submitting a Notice of Separation (S54)

     
    Per Dallas ISD Policy, all exiting employees are required to submit an official Notice of Separation (S54 Form). Dallas ISD no longer accepts the paper Notice of Separation (S54 Form). The electronic Notice of Separation (S54 Form) is accessible online through Oracle.
     
     
    To access the Notice of Separation (S54) Form in Oracle 

    1. Sign into Oracle using your district username and Oracle password

    *Note - If you do not remember your Oracle password, you can reset it using the "Password Reset" key on any biometric clock. Your password will be reset back to your social security number (no dashes).

    2. Select DISD Employee Self Service

    4. Select Human Capital Management Information
    Select Human Capital Management Information
    5. Select DISD Separation from Employment
    Select DISD Separation from Employment
    6.  Fill out the requested information and click Submit
