When submitting a nomination, the following materials must accompany the required nomination form.

• A short biographical description of the nominee's career demonstrating qualities that set the candidate apart from others;
  
  
• Mention of special honors, achievements, and recognitions including the approximate date of presentation and the name of the presenting organization;
  
  
• Other information that illustrates the nominee's “fit” with the criteria listed above.
