A look at the process

CONTACT
Contact the 504 Chairperson at the child’s school to request a Section 504 Evaluation.

SIGN
Parent and/or Guardian must sign consent giving permission for the evaluation to be done.

ATTEND
Attend the Section 504 evaluation meeting when invited.

RECEIVE
If the committee determines the child is eligible for services, the child will receive an accommodation plan with provided services.

REVIEW
A Section 504 Review meeting will be held each year to review and/or update the child’s Individual Accommodation Plan.

Your role as parent and/or guardian
Sign and ReturnSign and return the Notice and Consent to the school’s Section 504 Chairperson as soon as possible.
Give InputComplete the Parent Input Form as soon as possible and return it to the school’s Section 504 Chairperson.
Provide InfoIf your child has been diagnosed with a mental or physical impairment, be prepared to present updated documentation.
Be ThereAttend meetings held on behalf of your child. Contact the Section 504 Chairperson if you are unable to attend.
